Step 2: Complete and Sign KHC’s e-Waiver.
Before completing your e-Waiver, please answer the following questions:
- Are you completing your first e-Waiver (i.e. Step 2 of your Membership Registration)? Ignore any Guest e-Waivers you have created, when answering this question.
- Are you completing this e-Waiver in response to an email request from Member Services?
- Do your need to complete a new e-Waiver to correct information in your previous e-Waiver?
If any of the answers is Yes, continue to complete your e-Waiver.
If the answers are all No, don’t complete another e-Waiver. Member Services will be in contact with you, if necessary, to resolve any problem with your original e-Waiver. Click Membership Registration to return to the main Registration page.
Notice for Renewing Members: Even though we already have a waiver on file for renewing members, our KHC/FMCBC risk managers/lawyers advise us that we should get a new waiver each year.
IMPORTANT: Please read the following before completing your new e-Waiver:
- As the e-Waiver is a legal document, you must use your correct first and last names (no nicknames). Your screen (display) name can be different.
- Take a minute to check you have spelled your name and email address correctly before you tick the “Agree To This Document“. If you have a spelling error or transposed your first and last names, you will have to complete a new e-Waiver.
- After submitting your e-Waiver you will receive an email requesting you to click a button to verify your email address in the new e-Waiver. You must do this or else you will have to complete a new e-Waiver. You have three days to perform this verification. You don’t need to do another e-Waiver if you complete the email verification.
